Your Dress is Waiting For You!

We require setting up an appointment to come pick up your dress so we have it ready and waiting for you when you arrive. If you would like to try on your dress before taking it home with you, please let us know and we will allot the appropriate amount of time. At the time of item pickup, you will have the opportunity to inspect the item(s) prior to taking said item(s) from the boutique. Further, a waiver will be required to be signed releasing Off White of any liability pertaining to any issue that may arise upon the item leaving the premises.

*NOTE: IF YOU ARE DESIGNATING ANOTHER INDIVIDUAL TO PICK UP YOUR ITEM(S), YOU MUST FILL OUT THIS FORM IN ITS ENTIRETY PRIOR TO THAT INDIVIDUAL'S ARRIVAL AT THE BOUTIQUE. THIS FORM AUTHORIZES THAT INDIVIDUAL TO EXAMINE, SIGN THE RELEASE, AND TAKE YOUR ITEM(S) WITH THEM.
