OFF WHITE RECOGNIZES THAT NOT EVERYONE IS AVAILABLE TO BE MEASURED AT THE BOUTIQUE. IF YOU ARE MEASURED BY A THIRD PARTY, PLEASE SPECIFY WHO MEASURED YOU BELOW AND PROVIDE YOUR MEASUREMENTS. OFF WHITE WILL CALL YOU ONCE THIS FORM HAS BEEN RECEIVED TO DISCUSS SIZING AND LENGTH BASED ON THE APPROPRIATE DESIGNER SIZE CHART. NO ORDER WILL BE PLACED BY OFF WHITE UNLESS OFF WHITE RECOMMENDS A SIZE TO YOU. IT IS YOUR RESPONSIBILITY TO SEE THAT YOU DISCUSS SIZING AND OBTAIN THE RECOMMENDED SIZE FROM OFF WHITE.
PLEASE DO NOT MEASURE YOURSELF! THESE MEASUREMENTS ARE OFTEN INACCURATE AND CAN RESULT IN ORDERING THE INCORRECT SIZE. YOU WILL ALSO BE REQUIRED TO FILL OUT A SIZE WAIVER AGREEMENT WHICH CAN BE SENT TO YOU VIA EMAIL.

If you intend to wear a heel and you are taller than 5'7 , extra length may be necessary. This will be determined prior to placing the order and will be added by the designer during production. Please note, each designer has their own guidelines and pricing for standard length, extra length, and/or if there is availability to add extra length.

Off White recommends a size based upon your measurements at the time of your order by referencing the designer's size chart. We typically recommend the size associated with the largest measurement provided. If you measure at a size 16 or greater, you may be subject to a designer sizing fee. These fees vary by designer and by size. You are ultimately responsible for the size ordered and any alterations costs associated with making the dress fit appropriately. It is RARE to not require some sort of alterations to these dresses. Off White does not offer alteration services, but we are happy to make a recommendation.

When we are shipping gowns, we always make a signature required. By clicking the box below, you are giving FedEx permission to leave the package without a signature.

All orders must be paid in full prior to an order being placed with a designer. Off White requires that a credit card be maintained at all times. If you have chosen to have your item(s) shipped to you, the shipping charge will be assessed and paid at the time of the initial order. Credit card information is securely saved and disposed of following the wedding/wear date.

Off White will manually charge the credit card on file once this order form has been reviewed and a size has been agreed upon. Please note, your card WILL NOT be immediately charged upon submission and/or receipt of this form.

ORDER CONFIRMATION – The customer will receive an email receipt within 5 days of purchase from a team member of Off White. It is the customer’s responsibility to review the order and notify Off White within 24 hours of receipt if there are any errors or changes that need to be made prior to placing the order with the designer. If we do not receive any notification that changes need to be made, the order will be considered ready to be placed with the designer.

I acknowledge sale of goods and/or services in the amount of the total shown hereon. All sales, including deposits of any kind, are final. No refunds, returns or exchanges.

I agree with the designer, style, color and size for each item ordered. I understand that gowns are ordered based on a designer's size chart and are NOT custom made to my measurements. Off White is not responsible for a variance in my body measurements from the date of original purchase.
I understand that dye lots may vary from swatches and stock. I understand that colors of special order items and the color of beading, embroidery, and trim WILL NOT perfectly match the samples at Off White due to dry cleaning, oxidation and handling of samples.
I understand that when purchasing a floor sample item, it is sold in 'as is' condition. The cost of any repair, mending and cleaning will be my responsibility.
I understand that alterations (i.e. taking in, letting out, hemming, etc.) to the merchandise are most likely necessary and are not included in this sale. I understand that Off White does not perform alterations. I further understand that I will not hold this store liable for any alteration inaccuracies or errors.
I understand that Off White does not represent or guarantee a specific shipping or arrival date for my merchandise. Orders are placed to allow time for alterations prior to my event date. This does NOT apply to rush orders, in which my merchandise may arrive much closer to my wedding date. Off White is not responsible for delays beyond its control.
Curing is an option available to Off White within a reasonable time. In the event that Off White exercises its option to cure, it may do so up until the event date.
I understand that if the event is cancelled or I no longer desire the goods purchased, I am responsible for full payment of all merchandise ordered and/or bought out of stock.
Off White will notify me when my merchandise arrives. All balances are due and payable upon the arrival day of my goods/merchandise at Off White. The card on file will be charged upon receipt of those goods into Off White. In the event the card is declined, Off White may sell those goods within 14 days upon written notification VIA EMAIL ONLY to the last known email address on file. If I have not made full and final payment within that time, any and all monies tendered toward the purchase of the goods are forfeited.
In the event that I request Off White hold my goods or Off White has requested that goods be picked up, I will pick up those goods within 4 weeks. If I fail to do so, Off White may sell those goods within 14 days upon written notification VIA EMAIL ONLY to the last known email address on file. Any and all monies tendered toward the purchase of the goods are forfeited.
I understand that all sales are final, including deposits of any kind, and are neither returnable nor exchangeable. No Exceptions. I have read and understand all Store Policies and agree to the terms and conditions therein.
